This study evaluates the effectiveness of dual-arc volumetric modulated arc therapy (VMAT) planning, in which different gantry arc increments (GAIs) are applied to each arc, for prostate cancer treatment. Eighteen prostate cancer patients were included in this study. For each patient, three VMAT plans with different GAI combinations applied to each arc—30°/30° (Group A), 15°/15° (Group B), and 30°/15° (Group C)—were created for each patient. PTV coverage, OAR doses, and plan complexity metrics were assessed and compared across the groups. Group C (30°/15°) demonstrated the greatest mean dose reductions to the bladder, rectum, and left and right femoral heads—by 6.6 %, 15.7 %, 23.9 %, and 24.1 %, respectively, compared to Group A, and by 6.4 %, 12.8 %, 21.6 %, and 25.9 %, respectively, compared to Group B. Total monitor units did not significantly differ between groups, while the modulation complexity score showed a significant increase in Group C. Applying dual GAIs, particularly 30°/15°, significantly reduces OAR doses in prostate VMAT plans without compromising PTV coverage or introducing excessive plan complexity. This approach may be a viable strategy for improving clinical outcomes in prostate VMAT. © 2025 Elsevier Ltd
