Purpose : To describe measurement methods of choroidal thickness (CT) and vortex vein thickness (VVT) using ultra-widefield swept-source optical coherence tomography (UWF SS-OCT) system (Silverstone, Nikon Healthcare Japan, Inc, Tokyo, Japan) and compare the ratio of CT/VVT between pachychoroid disease and the other diseases. Methods : Patients who underwent UWF SS-OCT for vortex vein imaging and routine swept-source OCT (SS-OCT) (DRI Triton OCT, Topcon, Japan) were retrospectively reviewed. Agreement for subfoveal CT (SFCT) measurement between UWF SS-OCT and DRI SS-OCT was analyzed. Measurement of the VVT was performed on superotemporal and inferotemporal vortex ampullae using UWF SS-OCT.[JK1] To analyze the characteristics of vortex vein in eyes with pachychoroid disease, the parameters including SFCT, VVT, and SFCT/VVT were compared with those of diabetic retinopathy (DR), age-related macular degeneration (AMD), and normal control. The eyes with pachychoroid disease were defined as the eye with central serous chorioretinopathy (CSC) and pachychoroid pigment epitheliopathy on unilateral or bilateral eyes. Results : A total of 205 patients was screened. Finally, 216 eyes of 134 patients who met both inclusion and exclusion criteria were enrolled in the study. SFCT (431.2 ± 166.5 um) and VVT (239.9 ± 62.0 um) was significantly increased in the eyes with pachychoroid disease (28 eyes 20 patients) compared with those in the eyes with normal control (98 eyes 66 patients; 246.4 ± 89.1 um, 213.3 ± 48.1 um), diabetic retinopathy (46 eyes 26 patients; 247.4 ±116.6 um, 204.1 ± 49.3 um), and AMD (47 eyes 33 patients; 217.5 ± 95.2 um, 204.9 ± 56.6 um). SFCT/VVT and MCT/VVT ratios were significantly higher in pachychoroid group (1.83 ± 0.62, 1.65 ± 0.50) than those in normal control (1.20 ± 0.49; 1.11 ± 0.44 ), diabetic retinopathy (1.23 ± 0.53; 1.11 ± 0.45), and AMD (1.14 ± 0.64; 1.03 ± 0.55).[JK1] Conclusions : This study evaluated and described the VVT using UWF SS-OCT for the first time. Increased ratio of SFCT/VVT and MCT/VVT ratios might be useful new parameters for understanding of pathogenesis of pachychoroid disease.
