마을 단위 장애인에 대한 다학제주치의팀 중재의 예비 효과성 평가

Background: The right to health for persons with disabilities (PWDs) is essential for social integration, yet they face high unmet healthcare needs (17.3%). This study preliminarily evaluated a village-level ‘multidisciplinary primary care team’ (MPCT) model to address the limitations of existing single-professional care. Methods: Customized case management (medical, rehabilitation, welfare, nutrition) was provided to 41 PWDs in Ansan and Daejeon via an MPCT for approximately 6 months. Preliminary effectiveness and changes were analyzed using multidimensional indicators, including wellbeing levels, physical function (Instrumental Activities of Daily Living [IADL], World Health Organization Disability Assessment Schedule), service satisfaction, and social network expansion. Results: Interprofessional collaboration competency improved by an average of 7.9%, with a 14.8% increase in integrating knowledge and experience from different professions. Pain reduction was observed in 86.6% of participants, and IADL scores decreased by 6.9%, identifying the potential for improvement in activities of daily living levels. Regarding well-being, gains were found in life satisfaction (21.7%) and community support (18.2%), suggesting a potential expansion of social networks. While overall service satisfaction was high, participation quality was relatively lower. This difference was attributed to differences in provider-side operational methods and patient engagement strategies, suggesting the need for more sophisticated operational models to facilitate participation. Conclusion: The study suggests that integrated MPCT interventions in resource-limited settings show potential as an effective model for promoting health rights and well-being for PWDs. Future success requires establishing mechanisms to ensure the active participation of PWDs and building sustainable community networks.
