The purpose of this study was to evaluate the mechanical reliability of AMS hydraulic penile prostheses. Three-piece inflatable prostheses, 700 PPT, were placed into 10 patients, 700 CX into 19, 700 CXM into 45, and 700 Ultrex into 27 patients with a mean follow-up period of 142.9 months, 109.2 months, 43.3 months, and 70.1 months, respectively. Unitary prosthesis, Hydroflex were implanted into 32 patients (mean 137.3 months) and Dynaflex into 110 (mean 72.8 months). The mechanical failure rates of PPT, CX, CXM, and Ultrex were 10.0%, 10.5%, 8.9%, and 29.6%, respectively. The verified causes of the mechanical failures were: fractures at the junction of the input tube and cylinder in seven cases where some difficulties were encountered in placing the cylinders because of a small penis width, three fractures of the tubing near connector (2) and pump (1), three cylinder ruptures, and a pump malfunction. Hydroflex showed a higher rate of malfunction (18.8%) than Dynaflex (4.5%). It is concluded that the reliability of 700 CX/CXM and Dynaflex prostheses were higher than that of Ultrex and Hydroflex, respectively. A small penis may increase the likelihood of damage of the prosthesis due to the force exerted to place the cylinder of 3-piece inflatable and to deflate the unitary prosthesis (Hydroflex).
