Background and Aims: High-resolution magnetic resonance imaging(HRMRI) can provide information about wall structures and atherosclerotic plaque and be easier to investigate of progression of intracranial atherosclerotic stenosis(ICAS). This study aims to analyze of HRMRI follow-up(FU) data of middle cerebral artery(MCA) ICAS treated with medical management.Methods: From 2014 to 2018, we prospectively collected 22 patients with definite MCA stenosis of over 50% using HRMRI. All patients were treated with the statin and antiplatelet medication. All patients checked FU HRMRI after 1 year. The degree of stenosis and plaque burden were evaluated using time-to-flight MRA and cross-sectional images of HRMRI.Results: Among the 22 patients, 18 patients were initially treated with dual antiplatelet medication(DAPT), and 17 patients started to treat with the high-intensity statin. According to the HRMRI FU after 1 year, stenosis degree was decreased in 13 patients(59.1%) and increased in 3(13.6%). Plaque burden was decreased in 11 patients(50%) and increased in 3(13.6%). Initial plaque enhancement was observed in 17 patients(77.3%), decreased in 12(54.5%) and continued in 5(22.7%). However, statin intensity, use of initial DAPT, initial and FU of lipid profile were not related to changes of stenosis degree, plaque burden, and enhancement at 1-year FU results.Conclusions: Under medical management of statin and antiplatelet, ICAS of MCA for 1-year showed significant changes in FU using HRMRI. Approximately 60% of ICAS at MCA showed improvement of stenosis after 1 year FU with medical management. The FU of ICAS using HRMRI seems to be meaningful to evaluate ICAS changes as a potential stroke risk factor.
