Knowledge and Self-Reported Practices in Central Line-Associated Bloodstream Infection Prevention Among Nurses: The Mediating Effect of Perceived Barriers

Background: Central line-associated bloodstream infections (CLABSIs) in intensive care units (ICUs) increase mortality, prolong hospital stays and elevate healthcare costs. Equipping ICU nurses with the necessary skills to prevent CLABSI is critical for improving patient outcomes and reducing healthcare burdens. Aims: This study aims to investigate how perceived barriers mediate the relationship between knowledge and self-reported CLABSI prevention practices among ICU nurses. Study Design: This study employed a cross-sectional correlational design with a convenience sample of ICU nurses. Participants completed a web-based self-report questionnaire assessing the study variables. Mediation analysis was performed using the percentile bootstrap method to examine the role of perceived barriers in the relationship between knowledge and self-reported CLABSI prevention practices. Results: Data from 224 ICU nurses were analysed. Although knowledge was not a significant predictor (β = 0.014, SE = 0.014, p = 0.335), perceived barriers were identified as a key determinant of self-reported CLABSI prevention practices (β = −0.068, SE = 0.016, p < 0.001). Furthermore, perceived barriers were found to mediate the relationship between knowledge and these self-reported practices (β = 0.029, SE = 0.014, p = 0.037). Conclusions: Perceived barriers play a crucial role in the adherence of ICU nurses to self-reported CLABSI prevention practices. Relevance to Clinical Practice: As knowledge alone is insufficient, addressing these barriers through targeted interventions—such as training, structural support and policy changes—is essential for translating knowledge into effective practice and improving patient safety in critical care.
