Changes in 119 EMS Response Times at Dongjak Fire Station During Medical Disasters and a Survey on Active Paramedics' Perceptions of Introducing an AI-Assisted Transport System

초록

This study analyzes EMS response time changes at Dongjak Fire Station during medical disasters and proposes an AI-assisted transport support system. A Welch's t-test and Mann-Whitney U test on 63,392 transports(from 110,110 records) assessed delays, while a survey of 102 paramedics provided qualitative insights. Significant delays were confirmed across all crisis stages (p<0.001), with notable on-scene time increases versus pre-COVID-19, suggesting hospital selection difficulties as a possible contributing factor. Among paramedics, 57.9% rated the real-time bed information sharing system as inconsistent with actual conditions(median=2.0), and 78.4% relied on direct phone calls to secure beds. Additionally, 53.9% lacked knowledge of out-of-jurisdiction hospitals, compounding these difficulties. Medical crises increased overall EMS operation times, reducing efficiency, and the existing system exacerbated these challenges. With 71.6% endorsing the need for an AI-assisted transport support system, introducing such a system reflecting real-time treatment capacity is urgent, alongside legal and institutional frameworks to strengthen hospital data accountability and prevent unreasonable patient refusal.
